HIPAA Notice of Privacy Practices – For your reference — this does not need to be brought in to the office.
Acknowledgement of the Receipt of HIPAA Privacy Policies and Procedures – Complete, sign and bring to your visit.
Request to Transfer X-Rays to our Office – Complete, and give to your previous dental office to have them send us a copy of your most recent x-rays before your initial appointment.
